To facilitate the process of obtaining an incident report you must have the date of the incident, the address or location of the incident, and the owner's name. Charges for the incident report are based on a charge of 25 cents per page for pick up and additional $2.00 charge if faxed or mailed. To pick up a report, the location is 320 N.
